Title pretty much says it all. It's based on Normal Again, but instead of Buffy being pricked by the demon who can not be named without serious tongue twisting, it was Spike.
In the psych ward, Buffy is his doctor. But she's not a full-fledged doctor, I think she's in training and takes him on because she in a way pities him and thinks he can be helped. Giles (I think) is her mentor. Angel is in the dreamland world as Spikes (who goes by William) brother, who Will is deathly afraid of. It turns out that Angel placed him in the mental hospital because he tried to kill himself.
Back in the real world the Scoobies are trying to find the antidote to the poison, but Spike at first isn't lucid to take it, then he decides it's better being in a world where Buffy sees him as a broken man as opposed to a monster.
Dreamland world, you find out Angel has multiple personalities i.e Angel and Angelus, and it was Angelus who tried to kill Will. Angel refuses to get help, and Angelus takes over completely. He then attacks and taunts Will telling him to keep the secret or he hurts Buffy. Buffy unwittingly puts the pieces together after spending some time with Angel. Buffy then says she's going to report him and have him admitted or something along those lines. Buffy gets attacked by Angelus, to keep her from saying anything, and is dying, when Spike/Will goes to save her. He, thinking he is in Sunnydale and a vampire, tries to get her to drink his blood so she will turn. But is pulled away by orderlies who think he is the one to attack her in a psychotic rage and killed his brother during that rage. Buffy dies and Spike is placed in isolation, strapped to a table, while the devil, and Spikes "soul" fight for who has possession of him. At the very end, the Scoobies get Spike to drink the antidote. In the dreamland, the Spike strapped to the table starts vomiting up all this foul, putrid mess, almost like he is rotting from the inside. The dreamland Spike dies, and the devil is pissed because he has lost him, and walks out through the wall scaring the nurse who can see him.
Unfortunately the end alludes me, I don't remember if him and Buffy fully get together (sex and all), or if they begin a tentative relationship, or if they just stay friends (with her being forever grateful that he saved her from the poison)
I will also say that throughout the story Spike/Will talks to his inner self (soul) who gives him advice throughout, trying to get him to leave the dreamland world and return to Buffy who needs him.
