This one is an old one. It was loosely based on the movie The Crush with Alicia Silverstone and Cary Elwes. What I do remember about it, is that it's a past story set in the present (if that makes any sense), but the story is told in flashbacks ( I believe) as Buffy remembers her time with Spike (William) as she talks to her therapist. I also think Buffy is in a mental ward/hospital (not 100% sure though).
I don't remember how or why Buffy/Spike (William) got involved, but for some reason my mind keeps thinking he was her original therapist, and it escalated from there. I do remember that you never don't know what happened to Spike or why he was no longer her therapist or why he wasn't around, until the end, when Buffy is at his grave, I think with her therapist, and she says something like she didn't mean to kill him. And that he promised he would always be there for her. And then at the very end it describes her sitting in her room, looking at pictures of him, and letters he wrote, and she gets this kinda creepy smile, while her therapist and her mom are talking about how she is doing much better and that she would be released soon.
Now, since it's been a while since I read this, I'm not 100% sure about some of the details. So sorry... but I do remember it was loosely based on The Crush, and the whole story was pretty much one big flashback of her remembering him. I'm about 75% sure about him being dead at the end. If he's not dead then he was injured very badly and was definitely out of the picture.
