Spike has the Gem and Buffy's Pregnant

Buffy is pregnant and I think she knows the father is Spike (But he doesn't know). The Gem of Amara apparently makes Spike more human...I distinctly remember him visiting Buffy in the hospital, not knowing why she is there, and having to use the restroom because of the Gem of Amara. Does anyone remember this story or know where to find it.

Did this story have a fight with Adam and Buffy got really hurt, and she tells Spike before she passes out it's in the Peas?
