All human, buffy is pregnant, spike and Drusilla are siblings, and riley goes bad

Looking for a fic, where buffy is pregnant with spikes child. She is in the hospital and has to get prenatal vitamins, don't know if riley works in the hospital but he gets jealous of buffy and spike and switches her vitamins to make her sick. Drusilla is spikes sister, and she tells buffy that something bad will happen i think. And also spike and the family do know that she is special with psychic gifts. Also they are all human. Read this years ago, and have not been able to find it since.

Don't know if this is the one you want but it sounds a little like William's Girl fic, As You Wish.

Buffy does get pregnant in the fic with Spike's baby. She works in the hospital with Parker (not Riley) who is jealous of Spike. Riley is in the story but he is mentioned as someone with a crush on Dru. A crazy mental patient that thinks he's a vampire. Dru is Spike's sister and she's psychic in it. There is even a scene where Parker tries to give Buffy a bottle of prenatal vitamins but Dru knocks it out of his hands.
